Ways to Address Reach in Efficacy and Effectiveness Studies

During Efficacy Trials (Phase III Research), do these things to address Implementation:

  • Collect data on likely treatment demands.
  • Evaluate delivery of intervention protocol by different intervention agents (usually research staff).

During Effectiveness Trials (Phase IV Research), do these things to address Adoption:

  • Assess staff ability to implement key components of the intervention in routine practice.
  • Evaluate consistency of intervention delivery by agency staff who are not part of research team.

Common Challenge Encountered in Evaluating Health Behavior Interventions and Remedies

Protocols not being delivered as intended (type III errors) might be remedied by doing these things:

  • Assess if treatment (Tx) is too complicated, too intensive, or not compatible with other duties.
  • Involve potential (non-research) intervention agents in program design.

Questions to Ask about Implementation when Evaluating Health Promotion Programs and Policies

  • To what extent were the various intervention components delivered as intended (in the protocol)?
  • To what extent were the various intervention components delivered when the intervention was conducted by different (non-research) staff members?
  • To what extent were the various intervention components delivered when the intervention was conducted in applied settings?

Questions to Ask and Ways to Enhance Overall Impact

The Questions

  • How many staff within a setting will try the intervention?
  • Can different levels of staff implement the program successfully?
  • Are different components delivered as intended?

Possible Ways to Enhance Dissemination

  • Clear intervention protocols
  • Consider automating part or all of the program
  • Monitor and provide staff feedback and recognition for implementation

Guidelines and Multi-level Strategies to Enhance Dissemination and Public Health Impact

Individual Target Level

  • Publicize consumer guidelines

Delivery Site or Organization Level

  • Provide feedback about implementation
  • Involve many staff members
  • Automate the intervention for consistency

Systems or Network Level

  • Provide recognition, reinforcement, and rewards for staff
  • Fine tune, experiment, and revise
